It's more common to call your friend's relatives by their first name especially when more than one of them is present. Calling Hana's mom Tsuki-san feels less intimate than calling Hana by her first name. That's how their culture works.

Or Shinichi can call Tsuki 'Okaa-san' as in Hana's mom; that's also another common practice. But it has another meaning as in mother-in-law. Obviously, the first choice is safer for him.
